Registration

Delegates can register  by completing the enclosed registration form and returning it (by post or fax), along with payment, in the form of Demand Draft/Cash, to the Conference Secretariat.

Demand draft should be made in favour of “TRANSCON 2008” payable at Lucknow, with name and address of delegate clearly written on the back of demand draft. Additional registration amount for CME/Workshop /accommodation/ accompanying delegate should also be included in the same demand draft and the break up should be clearly mentioned in the appropriate columns of the registration form

Please complete one registration form per participant including any accompanying persons. Please photocopy the form if you require additional copies, or download from the website.
